I tried to reproduce both issues, but I seem to be unable.

1. Create a new project
2. add a file to the project, type objective-c header file, enter into name field: test
3. add a file to the project, type objective-c class file, name test
Result:
Check the List of header files in the project, now see two test.h files in the list.

Workaround: remove one from the PC.project file of the project, as they point to the same file.

4. add another file to the project, type objective-c class file, name test2.m

Result:
in the list of class files is a test2.m file, in the list of header files is a test2.m.h file. Either of these files is wrong.
