I can mostly confirm this on Fedora Linux with the current version from github. In most themes with a pattern background, balloons show up black. An expection is the Blackboard theme.

Preview-ref and comment pop-ups do work.

This appears to be an issue with these pop-ups having black text on a black background. If I insert a balloon where the balloon text has a different color, it does show the text:
<mouse-over-balloon|blah|<with|color|magenta|blah>|left|Bottom>
