I took a preliminary stab at this. I have attached an image of the results. I typed in the symbols from Table 21-1, and added two more entries at the end for glyphs that Table 21-1 did not include.

I did have to treat U+01D1A0 as a combining character and give it a width of zero, combining with the glyph that follows. Combining it on the last row of pixels with the previous glyph did not look good in sequences such as U+01D19C, U+01D1A0, U+01D19D. Positioning it one more pixel to the right made a noticeable difference.

I set the width of U+01D1A0 to zero by hard-coding an exception in hex2sfd, but because of the bug you just reported about double diacritics, I am going to see if I can implement a more general approach. I will describe this idea on that bug report.

The symbols from U+1D19B MUSICAL SYMBOL ORNAMENT STROKE-1 to U+1D1A5 MUSICAL SYMBOL ORNAMENT STROKE-11 are meant to join cursively to form full ornaments, but in Unifont Upper they do not join and are at inconsistent vertical positions. See table 21-1 of The Unicode Standard. These strokes should join to the right: 1, 7, 8, 10, 11. These should join to the left: 4, 5, 9. These should join on both sides: 2, 3, 6. Stroke 6 is so thin it might be necessary to implement it as a zero-width glyph.
