Well, I had forgotten that there is a second patch for bug #50030. That patch in combination with the patch for bug # 50126 solves this problem.

And second, it would be enough to limit just regex replacements to the forward direction -- normal replacements can go either way.

To reproduce, run 'echo 22222 >twos' and then 'src/nano --ignore twos'. Then type:
M-R M-R \B <Enter> x <Enter>

Then keep typing Y. The replacements never stop.

It seems there is no solution. When normal replacements work and don't get stuck, then regex \B will skip matches; and if regex \B does not skip anything, then normal replacements can get stuck.

If we could just scrap backward replacements... Allow backward searches, but allow replacements only to got forward...
