Hi,
After a wait of some years, I got back to this.
I have ripped out all references to the 'MLR ' language tag from the font, since even if the bug were fixed now, it wouldn't be reflected in many systems for years.
Let me know if you're aware of further problems.

The commit I just made to FreeSerif implements the two conjunct forms you pointed out: sa-rra-rra and na-rra. The first I had implemented before but broken; the second--I'm not sure it was ever implemented properly before.

I just committed changes to FreeSerif regarding the changes to the Unicode standard for zero and one-quarter.
And I see the (two) conjunct formation issues you poined out in your mail, and am working on them.
Again, besides these issues, the font seems to work well in Windows 7. We are still thinking about what to do about the problems in Linux -- we may have to resort to work-arounds.

Sure, I will try to test as an end user. I don't have much idea about font technology. I have added SMC mailing list in CC here.
Issue #1: Glyph for "Malayalam Digit Zero" U+0D66 should be changed

Part of what you are seeing is due to a Harfbuzz bug, which makes the "reformed" Malayalam script the default for Malayalam. Unfortunately, the SVN version of FreeFont makes a distinction.
Would you like to do a bit of testing for us?

I used the latest svn trunk version of FreeSans.sfd and generated TTF files and I found many rendering issues.
I used the test file available here:

I compared this against Lohit & Rachana fonts:
