Now all the issues raised by Neal, Zdenek, and the Wilkner table have been addressed, and I have done two iterations with the Velthuis table fixing broken conjuncts.
In latest SVN and snapshot.
Attached: an HTML version of the conjunct table, a PDF of the Wilkner table, and the x900 table from Velthuis.

Once I realized Wilkner's table was essentially the same as Neals, I made more headway. (Wilkner's docs are formatted using a font whose name is obfuscated!)
Some questions remain, but most of the problems are resolved.
In SVN and most recent snapshot. Attached find Neal's table made to be more like Wilkner's.
The ttra issue remains because the substitution ta+ra is done very early on, and would basically have to be un-done. Most of the other errors I saw are corrected.
Most of the problems had to do with double half-forms. I made a few new ones, and then made lookup tables to enable them. These are in force regardless of language, and the tables are for Sanskrit, so it may have broken Hindi with inappropriate combos.
The rakaar wasn't attached properly to cha. This is fixed.
Also fixed nga+ka+ssa+va which was broken in a recent commit,
and adjusted many half forms to connect better with next letter.

ttra and tkra are also in Wilkner's dictionary.
ttra: rather than half-ta+tra, it has tta+rakaar (a caron one I think)
ttka: rather than half-ta+tka, it has half-ta+half-ka+rakar (a bar one)
I wish I knew the rhyme or reason.

I noticed the guttural n + kSva combination still uses a virama for the n. All the others have kS combination underneath the guttural n.
(that is, dev_nga__ka__ssa.pres to replace nga+ka+ssa in nga+ssa+ka+va)

Also the t + n + y combo should build off the t + n form that is just before it just the double t combined with the y on the previous line.
Velthius font doesn't have such a half form for t+n.

These conjuncts don't appear in the Velthuis table.
They were presented by Neal Dalmonico on the XeTeX mailing list, and the errors reported by Zdenek Wagner. Remarks are by Zdenek, with HTML examples added.
1) ttva and ttya should have half tta + va or ya, it should be used also in Hindi

3) ptya should be composed of half pta + ya in Sanskrit, the current version is used in Hindi
