I tend to consider 10 years of support to be about right for base, but I know gui/back is different.
The thing is, I don't think it's unusual for gnu/linux distributions to include packages with versions several years old ... particularly when you are looking at quite stable packages with minimal changes.

So ... would it be worth simply adding:
#ifndef FC_WEIGHT_ULTRABLACK
#define FC_WEIGHT_ULTRABLACK FC_WEIGHT_BLACK
endif

fontconfig 2.4.1 was released five years ago, even for GNUstep standards this this old. And the actual change in fontconfig seems to have happened four years ago in release 2.4.91. (Which was just two releases after yours)

I would suggest that we see this as a limitation of CentOS and don't try to handle it in back. Otherwise we could require a minimal version of 2.5 for fontconfig.

Thanks ... I checked /usr/include/fontconfig/fontconfig.h and the define is not present.
Checking the installed packages I see:

$ rpm -q -a | fgrep fontconfig
fontconfig-2.4.1-7.el5
fontconfig-devel-2.4.1-7.el5

My quick hack to get back to compile was simply to replace FC_WEIGHT_ULTRABLACK with FC_WEIGHT_BLACK. I realise that probably won't give me the correct display but I don't know where I should be looking for it to show up.

'FC_WEIGHT_ULTRABLACK' doesn't come from the cairo library, it is part of fontconfig. You should find it in /usr/include/fontconfig/fontconfig.h. In the configure file we check for the presence of this header file and I haven't seen one without the definition of this variable. My header file (version 2.8.0) uses the following definition:
#define FC_WEIGHT_ULTRABLACK FC_WEIGHT_EXTRABLACK

This is on CentOS(RedHat) 5.7 with all the latest updates applied.

I guess the configure script is not checking the cairo version properly?
