I missed this. I have it decrement the score each time they get it wrong. They have three chances. I can subtract (3 - chances_left) to each wrong answer.

I don't know how the psychology of it would work in practice, but in fairness the score would be decreased more for repeated failures than for the first failure. This because repeated failure indicates a lower level of attainment.

So if the user has $x chances to get it correct would the score remain the same until $x is reached?

I have my students practice using the keyboard based games. They don't complain because the are just happy to play a video game in school!

My youngest so far is a four year olds playing LineOrSpace using the keyboard. This only involves two keys though.

NoteNameSpeedTest is similar to ReadNoteNames. It gives a time limit of 120 seconds. This can easily be changed for your needs. I like it better for beginners because I teach note reading via scale step at first rather than using "EGBDF" and "FACE". I use "ABCDEFGAB......" and "GFEDCBAGFED.....". This is why NoteNameSpeedTest only goes up and down by step. I am going to make a more advanced one that throws in thirds.

I am imagining that getting wrong gives the large X underneath as at present, but instead of presenting a new note to the user, it presents the note that was got wrong. In fact, it could just stay stuck in this until the user gets it right, although a further refinement might be to give the answer after, say, three wrong guesses at the same note.
The nine year old that reported this bug was mostly getting it wrong by miss-clicking, and was asking for pc-keyboard entry. Younger children might prefer the mouse clicking.

If I add this feature to some of the games what indicators should I use to show the user that the got it wrong and that the user has another try?

This bug was reported by a 9-year old trying the Educational->ReadingNoteNames program for real. The program would ideally give, say, two goes and then say what the correct answer is.
