I think it means that re-generating can take many iterations, and, currently TeXmacs can't tell how many iterations are needed and so might not re-generate enough times.

Just for the record, the documentation explicitly states that this information may be rebuilt (See the help item "TeXmacs documents"):

"Even though this information can be automatically recovered by the
typesetter, this recovery requires several passes. In order to make the
behaviour of the editor more natural when loading files, references are
therefore stored along with the document."

What is meant with "more natural when loading files"? Does this mean that simply regenerating the section is not a solution?

Confirmed: the section <\references> is not rebuilt even when clicking on "Update all" and then saving the document.

The structure in memory should be updated every time a label or ref is changed and maybe the on-disk version deprecated: I don't understand why it is needed, since it can be rebuilt each time the document is opened.

As a test, simply commenting out the two lines responsible for saving the \references section to disk inside tm_file.cpp, tm_data_rep::make_document() allows this while seemingly not affecting performance (maybe load times?)

This would also help with issue #12968.

References to deleted labels still contain their value.

1. Create a section in a new article.
2. Add a label, foo, in that section.
3. Create a reference to foo.
4. The reference reads as "1" (the section number)
5. Delete the label foo, or rename it to bar.

The reference still reads as "1". Even after doing an Update All, the reference remain.

This error becomes quite apparent when you export the document to LaTeX, and LaTeX then complains about undefined references.
