I'm evaluating the project you submitted for approval in Savannah. While doing so I have noticed a few problems which are described below.

The address of the FSF has changed, and is now:

(alternatively the GNU (L)GPLv3 standard notices include different wording that do not mention the FSF snail mail address - see http://www.gnu.org/licenses/gpl.html#howto)
