User story: Users want a way to automatically generate pdf
files for all lives in the census. They also want 'InsuredName'
embedded in the filename automatically, whenever it's specified
in the input.

Analysis: Adding a new feature: 'Print all lives in census
to pdf' should automatically print each cell in the census
to pdf, then name each file as it is today plus 'InsuredName'.
Today, the naming convention is this:

  SavedCensusFilename.000000001.pdf
  SavedCensusFilename.000000002.pdf
  ...

and users now want this:

  SavedCensusFilename.InsuredName.000000001.pdf
  SavedCensusFilename.InsuredName.000000002.pdf
  ...

where 'InsuredName' is exactly what's in that input. If
nothing is in that field, then the filename should be
the same as it is today. Composites should retain the
current naming convention. Also, it makes sense to retain
the numerical extension because some users may use a
census file to demonstrate multiple scenarios for the
same individual. In that case, the numerical extensions
will keep each pdf unique.
