This happens to me too fairly regularly. I think iTerm2 is tickling something in screen.
I used screen in xterms on this mac for years with no problems.

screen -v
Screen version 4.00.03 (FAU) 23-Oct-06

(I know it's old, it's the one with OSX)

iTerm2 Build 1.0.0.20140629

It usually happens when I option-click to paste in some multi line text.

Yes

It's the ANSI code for up arrow. I might have hit it once or twice, but I didn't hit it hundreds of times.

I don't remember exactly what I was doing. I spend most of my time in bash and emacs.

This happens to me a few times a month. I built 4.2.1 on Apr 30. This is the first time it's happened since, but I'm sure it will happen again.

I was working in screen, and it stopped responding. The master SCREEN process is using 100% of a CPU.

The problem appears to be that fore->w_inbuf is full:

(gdb) p sizeof(fore->w_inbuf)
$20 = 4096
(gdb) p fore->w_inlen
$21 = 4096
(gdb) p f
$22 = 0

Therefore, no data can be copied into w_inbuf from *bufpp, and the loop at process.c:1837 never exits.

The only way out is to forcibly kill the screen process, which loses all of my session state.
