Translating the contextual help is a quite subtle issue indeed, since a lot of this has been programmed in Scheme. I recently started experimenting with documents that contain some automatically generated content. This puts more serious constraints on the markup and the interaction with scheme, but would have the advantage that translations become easy to achieve.
