At the beginning of a list item, when the item symbol is still surrounded by a focus frame, typing two double quotes does not result in smart quotes: both of the double quotes are typeset as closing quotes.
To get smart quotes one needs to first type a space.
I suspect that the rule that quotes become smart only after a space or at the beginning of a paragraph needs to include also the case of the "beginning of a list item".
