The anchor mlm-reph is not defined for conjuncts like mal_kaka.

Now I understand why I was so perplexed, and why I kept re-fixing this.

This mark is meant be encoded to precede a consonant cluster, as opposed to other marks, which follow other marks.
See for example "Proposal to encode Malayalam Dot-Reph character" ISO/IEC L2/09-245R
[reph is discussed in several proposals -- there was contraversy: see https://en.wikipedia.org/wiki/Malayalam_(Unicode_block)]

Like the reph of other scripts, should it not be re-ordered by the font shaping engine to come after the consonant cluster! So
1) no 'reph' table is used in Malayalam
2) a 'mark' anchor will suffice to place the reph over a preceding consonant cluster.

Fix in SVN. Thanks for making me look at it again!

Malayalam has been out of commission for some time.
It's a long and disturbing story. I'll get to it soon, I hope.

U+0D4E MALAYALAM LETTER DOT REPH is placed incorrectly in FreeSerif and FreeSerif Bold. It should be placed above the following letter, but instead it is placed after the following letter.

FreeSans places this character correctly by using anchor points.
