I have a quick bug-report for
your book - the conversion of the pessimal bounds for n and h in 5.1.1 are
incorrect. h < 1.44 lg(n+2) - 0.328  inverts to n > 2^((h+0.328)/1.44),
you seem to have dropped the outer of brackets in your rendering of the
latter.
